We Accept:

  • Blue Cross Blue Shield PPO

  • UnitedHealthcare

  • Aetna

  • Cigna

  • Medicare (not medicare advantage)​ with secondary supplements like BCBS or Aetna

We Do NOT Accept:

  • Any patients with Prime Healthcare (it may have BCBS label on it, but we do not accept it)

  • Any HMO plans

  • Any Medicaid plans

  • POS plans.   These plans are hybrid.  ALL even with referral are out-of-network .  ALL have high deductibles. ALL cost due at time of service. ALL do not cover preventative services like a skin check to comply with federal law

  • Medicare advantage plans

  • Humana

  • Meridian

  • Community plans

💳 Payment Policy

  • Card on File Required for all patients (credit, debit, HSA/FSA)

  • No cash or checks accepted

  • Deductible Plans: If your plan has a deductible and no copay

    • While you pay premium payments for insurance, they do not cover any service until deductible is met.

    • New patients: $200 collected on day of visit (this is if you have a valid insurance)

    • Follow-ups: $100 collected for the follow-ups (this is if you have a valid insurance).

    • Surgical visits: $500 deposits

    • Remaining balance billed after insurance processes

      • If the visit fee billed is $275, insurance may adjust to $190.  Total remaining to be billed later (or credited). 

  • Copay + deductible plans: only copay collected at visit

  • Procedures: Billed to insurance; these apply to deductible: will discuss before the procedure

  • Refunds: Any credit is refunded to the original card after claims are processed

  • Self pay: $275 (new), $175 (follow up). This includes those for medical or cosmetics.

  • We do not offer free cosmetics services, the fees are not applied to future services

Pathology (if self pay): $150.  This is just for H&E stain.  The pathology lab will call before performing further stains.

⚠️ Additional Notes

  • Not all insurances cover a skin check plus another service on the same day (Aetna, Cigna, UH some BCBS)

  • A skin check may not be covered at 100%. Check with your carrier (HSA plans are not covered under federal law regardless of IL law)
