1. Driver license or state ID card is required.

2. Credit card will be required to be put on file:

• This will cover for any  copays, deductibles and out of pocket expenses.

• Insurances do not cover any cosmetics.

3. ALL plans with an active deductible will be charge on the first visit $150 to cover the initial costs​.
