We Accept:

• Blue Cross Blue Shield PPO

• UnitedHealthcare

• Aetna

• Cigna

• Medicare

• Other PPO plans

We Do NOT Accept:

• Any patients with Prime Healthcare (it may have BCBS label on it, but we do not accept it)

• Any HMO plans​

• Many of the POS plans of BCBS/United etc.: we do not accept. These are tricky plans and even PCP referral are not an acceptance of the insurance. Full amount due at time of service (considered self pay rate with deposit of $275 for new patients and $175 for follow-up)

Before Your Visit

1. ID Required: Bring a driver’s license or state ID.

2. Credit Card on File Required:

   • Used for copays, deductibles, or out-of-pocket costs.

   • Stored securely; charged 7 days after first statement.

   • Statements available via Patient Portal or by request.

   • Declining to store a card? Deposit required:

     • $275 for medical visits with any remaining to be billed same day or later pending insurance

     • $250–$500 for cosmetic/surgical visits with any remaining balance billed same day

   • Cosmetic consult: $275

3. Deductible Plans: If your plan has a deductible and no copay.  This means insurance will not pay any portion of the visit until that deductible is met.

   • New patients: $200 collected on day of visit

   • Follow-ups: $100 collected for the follow-ups. 

   • For example, $3000 deductible without copay​

     • If the visit costs billed is $260.  Insurance may reduces to $190.  Total remaining to be billed later (or credited).

   • Those with a copay will get charged only a copay unless a procedure is performed and you have a deductible. Estimated costs are on here.​​​​​

• We do not accept cash or checks.

• Check with your insurance provider for coverage, copays, and deductibles. We cannot answer these questions.
