Common visit codes with estimated costs based on contracted insurances:

99204 (new patient $200.  MEDICAL DECISION MAKING LEVEL 3/4 OR 45 MIN VISIT)

99214 (established patient, $145)

11104 (biopsy of lesion, small sample taken from lesion, $150)

11401(excision of lesion, removal of entire lesion on trunk. varies on size and location, $200-$700 )

11900 (injection for cysts or inflamed areas, $90-$150 )

17110 (removal of benign lesions other than skin tags/cutaneous vascular lesions, up to 14 lesions, $125-$175)

PLEASE NOTE: ANY DEDUCTIBLE PLANS will BE CHARGED $75 ON INITIAL VISIT to cover initial costs.  The remaining balance will be collected using card on file 7 days after processing with insurance company.

Any credit on the account is refunded after processing claims with original CC used.
