• Blue Cross Blue Shield PPO

• UnitedHealthcare

• Medicare (not medicare advantage)

• Other PPO plans

We Do NOT Accept:

• Any patients with Prime Healthcare (it may have BCBS label on it, but we do not accept it)

• Any new Cigna or Aetna patients

• Any HMO plans

• No Medicaid patients at this time (Oct 1 2025)

• POS.   These plans are hybrid.  ALL even with referral are out-of-network .  ALL have high deductibles. ALL cost due at time of service.

💳 Payment Policy

• Card on File Required for all patients (credit, debit, HSA/FSA)

• No cash or checks accepted

• Deductible Plans: If your plan has a deductible and no copay

  • $200 collected at visit if no copay listed (some plans are copay AFTER deductible is met, ask your insurance before booking)

  • Remaining balance billed after insurance processes

  • For example, $3000 deductible without copay

  • New patients: $200 collected on day of visit

  • Follow-ups: $100 collected for the follow-ups. 

  • If the visit costs billed is $260.  Insurance may reduces to $190.  Total remaining to be billed later (or credited). 

• Copay + deductible plans: only copay collected at visit

• Procedures: Billed to insurance; these apply to deductible: will discuss before the procedure

• Refunds: Any credit is refunded to the original card after claims are processed

Pathology (if self pay): $125.  This is just for H&E stain.  The pathology lab will call before performing further stains.

⚠️ Additional Notes

• Not all insurances cover a skin check plus another service on the same day (Aetna, Cigna, UH some BCBS)

• A skin check may not be covered at 100%. Check with your carrier (HSA plans are not covered under federal law regardless of IL law)
