Deductible Plans: If your plan has a deductible and no copay

$200 collected at visit if no copay listed (some plans are copay AFTER deductible is met, ask your insurance before booking)

Remaining balance billed after insurance processes

For example, $3000 deductible without copay

New patients: $200 collected on day of visit

Follow-ups: $100 collected for the follow-ups.

If the visit costs billed is $260. Insurance may reduces to $190. Total remaining to be billed later (or credited).

A skin check may not be covered at 100%. Check with your carrier (HSA plans are not covered under federal law regardless of IL law)
