• Any Medicare Advantage plans

• Any patients with Prime Healthcare

• Any HMO plans

• No Medicaid patients at this time (Oct 1 2025)

💳 Payment Policy

• Card on File Required for all patients (credit, debit, HSA/FSA)

• No cash or checks accepted

• Deductible plans:

  • $75 collected at visit if no copay listed

  • Remaining balance billed after insurance processes

  • for example, $3000 deductible without copay, $75 collect on day of visit.  The visit costs billed is $260.  Insurance reduces to $190.  Total remaining is $190-$75= $115

• Copay + deductible plans: only copay collected at visit

• Procedures: Billed to insurance; may apply to deductible

• Refunds: Any credit is refunded to the original card after claims are processed
